Women who've died in prison who could still be alive today, according to the charity Inquest, if successive governments had delivered on promises to change the system.
New data shared with this programme, reveals a third of all female deaths in prison between 2018 and 2024 were self-inflicted.
The Ministry of Justice told us they accept prison is "not working for most women," and they're looking at diverting more away from custody.
