A long awaited report into the Hillsborough disaster says there were "fundamental failures" by police on the day of the FA cup semi final match when 97 fans were crushed to death in April 1989.

The Independent police watchdog also found "concerted efforts" to blame fans for the disaster.
Their report said 12 former police officers would have faced gross misconduct cases although they all retired before investigations began.
