12 more prisoners were released by mistake in the last month, with two of them still at large, the UK Justice secretary David Lammy has said.

Mr Lammy, who's due to outline major reforms to the justice system later today, said there was a "downward trend" in the number of accidental releases.
But opposition MPs claimed there was a "dangerous level of incompetence" within the system.
