Southport attack ‘could have been prevented’ - Inquiry finds
The report from the first phase of the inquiry into the Southport killings has said the attack which left three young girls dead "could and should have been prevented".
The inquiry found that the killer Axel Rudakubana was "clearly" an extreme danger. But it said that his parents and a series of state agencies had failed to act sooner over his "escalating behaviour".
Alice da Silva Aguiar, nine, Bebe King, six, and Elsie Dot Stancombe, seven, were killed during an indiscriminate knife attack at their dance workshop in July 2024.
