An inquiry has found there was a “complete failure” to protect babies from Lucy Letby, who is serving 15 whole-life terms for killing seven babies and attempting to kill seven more at Countess of Chester Hospital.
Babies died at the hands of Letby because senior managers didn't do their jobs is the damning conclusion from the Thirwall report, detailing the systemic failures of yet another NHS trust.
Lady Justice Thirwall found senior managers took too long to call the police and they also devised an aggressive plan to remove the doctors who raised the concerns.
The lives of three out of the seven babies could have been saved if the trust had been more concerned about patient safety and less about its reputation.
Thirwall made 17 recommendations, including cameras in cots in neonatal units, and Health Secretary Yvette Cooper has promised a new way to monitor whether recommendations from NHS inquiries actually take place.
