Christa Pike's execution was halted and she was taken to hospital when she was still showing signs of life after receiving two lethal injections.
Governor Bill Lee said he was "highly disappointed".
50-year-old  Pike was sentenced to death in 1996 aged 18 for murdering her 19-year-old schoolmate Colleen Slemmer.
Including Pike, there are 40 people currently on death row in the state of Tennessee.
It is the second failed attempt to execute someone in the U.S. state this year, after prison officials failed to find a suitable vein for the lethal injection of inmate Tony Carruthers, who was also convicted of murder.
